- In the landing page choose the preferred language from English and Spanish.
- Enter the 17-digit survey code provided on the receipt in the first field.
- Provide the date of your visit from the dropdowns and click on start.
- If you do not have the survey code printed on the receipt click on the link provided above.
- In the following page enter the store number in the first field as provided on the receipt.
- Provide the date and time of your visit from the dropdowns.
- Enter the ticket number as given and click on start.
- After entering the survey, you will be asked regarding the experience of your visit and the behaviour of the staff, availability of the products, services offered and the overall satisfaction.
- Answer all the questions honestly and correctly as it will help in the improvement of the products and services provided in future.
- Give your feedback in the comment boxes provided and click on next.
- After answering all the questions, you will be asked to enter the sweepstakes.
- Agree to the notification and provide your personal details such as name, phone number, email id and complete postal address and click on next.
- Congratulations you have successfully entered into the sweepstakes and you will be notified once you win the prize.
